Tongkat ali or long jack [ Scientific Name Eurycoma longifolia]
Several years ago the western world discovered an eastern plant called “Tongkat ali.” This plant is a tree reaching about 12 meters high, generally found in the wild in Southeast Asia. It is traditionally used for the treatment of malaria, high blood pressure, fever and loss of sexual desire. However, the plant is a traditional secret of Malays as a booster for strength, stamina and possibly immunity. The first documentation on tongkat ali was done in 1939 in the Dictionary of Malaysian Medicine. It is the roots of at least a 7-year old plant that is harvested. It is said that tongkat ali is five times more potent than ginseng.
Tongkat ali is now a very popular natural product for increasing libido, enhancing orgasm, voluntary control of sexual performance, enhanced energy and stamina; providing the natural solution to erectile dysfunction in a gradual and safe manner. Animal studies have shown that tongkat ali produces androgenic (male hormone property) effects similar to testosterone treated animals. It can also produce arousal, libido and sexual motivation in sluggish, old rats Tongkat ali appears to stimulate the availability of endogenous testosterone (natural testosterone content of the body).
Those men who are getting old are actually having a 10% decline of available active testosterone every 10 years (that we get older). Generally, the decline becomes three times by their 60th birthday. The decline is gradual and may start as early as age 30 years. So by the time males become 80 years old the testosterone could have declined by as much as 50-70% relative to the level or amounts when males were 20 years old. This is a condition called andropause. However this does not mean that a large amount of testosterone is not present, it is, but it is not available. Most of the testosterone are bound in the blood stream and contributing to this is the fact that with age the testosterone produced by cells in the testis is also progressively declining. Tongkat ali is the natural supplement that has the property to effect a complete improvement in testosterone level. This improvement in testosterone is reported to be achieved in two weeks of intake; bringing the back “normal” level that has declined with age. These imply that tongkat ali acts naturally in the body and achieves a level of testosterone needed for a successful male sexual performance. For the men who are infertile due to problems with the quality of their sperm, Tongkat ali can raise the amount of available testosterone that will help in improve the morphology, activity (motility) and count of sperms.
While studies and popularity of tongkat ali have centered on enhancement of male sexual dysfunction, as an aphrodisiac, there have been in-vitro (cells in culture, i.e., outside the body) studies on the action of plant against lung and breast cancer as well as anti-malarial activity (the original traditional use). This could be attributed to the presence of phytochemicals called quassinoids and diterpenoids. It also contains an anti-oxidant called super oxide dismutase.
For these reasons the plant is considered an important economic and medicinal material of Malaysia. Today, tongkat ali is part of collaborative investigation of both Malaysia and the US Massachusetts Institute of Technology and also part of the database documentation of the International Center for Science and High Technology and UNIDO (United Nations Industrial Development Organization).

Tongkat ali is available in the Philippines in the form of capsule with brand name of Enduranz. It is an over the counter food supplement (no need for a prescription). You can buy this from leading drug stores like Mercury Drug, Watsons and Rose Pharmacy. The dose is 1 capsule twice a day. One capsule in the morning and another 1 in the evening. The medicinal property is gradual, 1 week on the average, before desired effects are obtained. However, it may take longer in others.

The usual dose for Enduranz is 1 capsule 2x daily. 1 week of taking it and then shifting to “maintenance” is too short. Usually, the effect after the first week should be better at the second or third week and there is no “maintenance dose” after you feel the benefits taking Enduranz. I would not compare Enduranz with others. The other components of Enduranz are intended to provide the health benefits of having stamina and vigor that would otherwise be lost as one ages. Dear Sir,
i am a 28 year, i have a problem with my health, but unfortunately i dont have enough money to go to the doctor. My problem is regarding odor. i always have cold, my friend said that there is something like bad smell when i inhale or exhale. I already had my dental cleaned but the unwanted bad smell doesnt disappear. when i eat fish or other meat, they can even smell the scent of fish even an hours after meal. on my younger age, i even had a sinusitus, last two when i had a money, i went to hospital for an X-ray and foound out that my sinus is clear.

Tongkat ali makes testosterone bioavailable. The testosterone produced by the body is in circulation but it becomes bound with globulin and so does not exert its effect on target cells. Tongkat ali unbinds the globulin-testosterone “link” allowing the testosterone to act. If one does not or cannot use testosterone because of it binded state, the negative feedback is sustained, i.e., the body thinks there is still plenty of testosterone.

Dear Dr. Emil Aligui,
I am 31 yrs old and experiencing changes with my erection. It’s less frequent as compared before.
I already had tests like FBS, Oral Glucose Tolerance Test, Stress Test, 2-D Echo (i also have complained about palpitation but cardiologist said i’m just conscious about it), and Glycosilated Hemoglobin. All of those test turned out to be normal. My BP is also normal 120/80 sometimes it’s 110/80. I came out to a conclusion myself that i’m not diabetic nor hypertensive in which are factors of ED according to the articles that i’ve read. What are the other possible causes of ED if one is not diabetic nor hypertensive?
TV advertisement tells that Enduranz are for men 40 yrs and above. Since i’m only 31, is it safe for me to take it? In your previous posts here you said that dosage is 1 cap two times a day, could i also do the same? I’m also experiencing difficulty in breathing and easily to get weak. Cardio said it’s not asthma and my chest xray doesn’t show any abnormality. He just prescibed arcoxia.

Please be careful about taking arcoxia beyond doctors supervision. It is a powerful analgesic and has been reported to increase the risk of a heart attack. To a certain extent, tongkat ali can improve muscle size and that would include penile size. However, the muscle size is influenced by the body’s own testosterone so its natural. Exercise like kegel exercise have also been claimed beneficial. As regards the food supplements you mentioned, no information on safety have been evaluated, so be careful about this. Penis pump have been recommended to increase penis size but this can harm the blood vessels and may result in damage. Consult a urologist for mechanical methods of increasing penis size because they are the experts.

Dear Doc, Emil,
I am already 55 years old, last year I was diagnose to have a nodule on my Thyroid, by an ENT and Neck specialist. Nodule consist of part mash and solid and part is fluids. Doc recommend to be remove by operation, Thyriod fuction test reveal its normal. another lab. test result its not cancerous. I see another Doc. specialized in thyroid, upon seeing the ultra sound results he asperate the fluids for few times and put some midcations and he told me it will be ok. Another doc specialized in throat and neck recomends to be removed by operations.
After a year or just few days again I went for ultra sounds and reveal that I have a multiple nodule in different sizes. the same doctors who recomend to be operated told me now that it is better just to monitor because if it will be operated the whole thyroid will be remove.
Now my question is, Is there any herbal that I could used to totally remove all those nodule?
A friend of mine told me to boil a narra bark and drink the water just like tea and those nodule will disappear. How true is this and if it is ok tell me the right method.
I know this is not related to the above subject..

Your case is complicated and it would be important to know the histopathology report on what you really have. Narra has been claimed to be beneficial in tumors but no clinical trials have been done. I noticed that in other international websites that talk about tongkat ali, they do not suggest continuous use of the product. They usually advice 1 month on then 1 month off dosing. There were also other dosing schemes but most of them advice against continuous use.
Here in the Philippines, enduranz is taken 1 capsule 2 times a day. Can you comment on this?

Tongkat ali preparations that are available abroad would have 400mg, however, reports have indicated that this is too high and not necessary. It is believed that the negative feedback mechanism for testosterone may be affected with this dose and an on/off schedule of taking tongkat ali is recommended. There is a theoretical basis for this but there have been no studies to measure free testosterone among indviduals taking tongkat ali in a research study. I would then recommend people to choose the preparations that would be lower than 400 mg/day in order to be on the “safe side.”
Enduranz has a lower content (<400mg) even with 1 capsule 2x a day. With Enduranz, the intake can be flexible because it contains an extract that would give less than 400mg even with 2 capsules per day, but with enough gradual effect to meet the natural bioavailability of testosterone.

I watched Rated K and the situation of the couple having a hard time to have a baby for 3 years now is actually the same with us.
My husband has lower sperm count with poor mobility and mortality due to mumps he had before that caused his one ball (testes) became small. The doctor gave us a L-Carnicor 3x a day but then it is very expensive and this would give some bad effects to his liver after a year his sperm count still the same though the quality has improvement.
My question is… Is there any side effect if he would take it continuously even for more than a year or more without any gap of consumption?

Yes, you can take Enduranz at 1 capsule daily before breakfast, since your young. I would not recommend sexual aids such as “bolitas” or “humps.” These can may cause scarring tissue to grow and become problematic. Your technique is your most important asset. Condoms can protect you from sexually transmitted viral/bacterial infections given that you use the condom properly and there is no possibility that you have a wound (or skin break) in your sex organ. Oral something (OS) has a risk of infection with human papilloma virus which can lead to oropharyngeal cancer, you must be very careful with OS. Kissing? It depends, but if the partner is infected with herpes, it can be transmitted.
On being a tough guy, I would not be in position to give you advice because that is a behavior with a lot of ways to describe or define. I suggest you consult Dr. Margie Homes. On being under the physiologic control of testosterone, virility, manhood, etc. Enduranz can definitely help.

From Doc Emil: Although tongkat ali (Enduranz) is recommended for men, there is no harm if women take it. It has use as a source of antioxidants and can also help the woman’s hormonal cycle. We have very limited information on effects on women; if you take it, I would say 1 capsule daily with rest periods in between a 2-3 weeks which should synchronize with your hormonal cycle (women also produce testosterone which influence sex drive).

From Doc Emil: Premature ejaculation can be managed by a combination of physical development of the muscles in the pelvic area (the pubo-rectalis muscle) and conscious control of ejaculation by the same muscles. As to over masturbation, anything in excess is not good which could have resulted in muscular fatigue of your penile muscles -this is would normally be reversible. Enduranz can help make available testosterone which your body actually produce and stimulate the testis to produce more testosterone as this hormone gets used (physiologic feedback). im 27 years old. still young but my performance in sex change when i got married when i was 22. i now have two kids. i notice that my penis reduce on its size and i begin to experience ejaculation too early than when i was still single. my libido is still very high when i was in high school i often masturbate 3 to 5 times a day to satisfy me. when i was in college i was 17 when i have my first sex. I can have an erection 4 to 6 times. but now after 1 or to 2 i can no longer have an erection.
i take endurance for three weeks then i stop when i am not satisfied with the result. my questions are:

From Doc Emil: If you are overweight, take measures to reduce, this is important. Take time to have wholesome activities. Most people would have been satisfied with Enduranz at 1 capsule before meals 2x daily which gives results in 2-3 weeks time but some may take longer. Having said that, there is no harm if you take it 3x daily. Enduranz is a food supplement that acts on making the body’s own testosterone “bioavailable” or unbounded and may take time especially if you are a case with lots of testosterone in bounded state (testosterone in a bounded state is useless testosterone). It is slow acting because it is natural and not a stimulant, try using Enduranz again for 6 weeks. Take Vitamin E 400 IU daily plus folic acid. Centrum tab is good but you need more Vitamin E aside form Centrum. Reduce your intake of fat and animal protein, take soy milk instead, make an exercise plan such as jogging, brisk walking or bicycle -this would improve the overall blood circulation. If you reduce body weight your penile size will likely increase. There is no recommended way to penile exercise; if your concern is penile size, it would be best to consult a urologist.

From Doc Emil: Apoptosis is cellular “suicide” so you can imagine breast cancer cells get destroyed by themselves with tongkat ali. Some cancer cells when combined with certain substances, such as those found in tongkat ali, become depleted of vital cellular support and die. Cytotoxic means “poisonous” or lethal to cells… this would mean that tongkat ali is lethal to cancer cells.

I am wondering if tongkat ali makes testosterone “more available” binding by SHBG that means also cortison will be “more available” because cortison binds also on SHBG. Conclusion: elevates testosterone but also cortison. In fact some studies claim that tongkat ali elevate the cortison level.

From Doc Emil: Your case is surgical. Go back to your OB-Gyne for proper advice. Essentially, the fallopian tubes need to be un-obstructed and egg from the ovaries need to be intact, in transit to the fallopian tubes -so any obstruction will prevent egg transit and subsequent fertilization.

As for your permature ejaculation, you can try Kegel exercise. I’m 61 years old male with low level of total testosterone (272 ng/dL) and free testosterone of 51.4 pg/mL. I was prescribed with 1.25 g/day of Androgel cream. Can I take Enduranz at the same time with Androgel? Also, do you have any products containing micronized natural testosterone (natural based formulation)?
Also my wife was diagnosed with clustered calcification in the breast. Don’t know whether a biopsy is needed. Can you recommend a product that will help cure her problem.

From Do Emil: Some consider 200 ng/dl testosterone to be low normal and free testosterone > 80 pg/ml as normal. You can take Enduranz with Androgel but be sure to recheck free testosterone after 6 months to see if you have to adjust on Androgel. The reason for this is that Tongkat ali will act to release your own body’s testrosterone and being low normal you might just be able to un-bind the testestosterone through Enduranz.
As for the breast calcification, the radiologist should be able to differentiate if the calcification is benign which will not require biopsy. The significance of calcification when seen on x-rays, be it on the breast or the large blood vessel such as the aorta is now thought of as a risk factor for cardiovascular disease; usually indicating atherosclerosis.

From Doc Emil: Tongkat ali can help your sperm to increase and become more motile but this will depend also on the intake of vitamins and minerals: Vitamin A, C, Zinc, Manganese. Check your multivitamin preparation and make sure you have these vitamins and minerals included. You must also have good stress management program because sperm health can be stress related. Have adequate sleep. Most fertility problems are managed by 1 year of close follow-up with a urologist (for males). Don’t miss the check up with your doctor.

Im 29 years old & my husband is also 29..5 years on married still no baby…We visit our OB last two months para mafind out kung sino may problema sa aming magasawa. Is it true that hypertensive medication that is being taken for maintenance of high blood pressure such as Lisinopril (10mg) lowers sperm count and affects sexual libido among men?

i was under medication for 9months with PTB i have taken medicines such as FIXCOM 4 for 3months and FIXCOM 3 for 3 months and RIMACTACID 300 for 3months total 9months, is there any known side effect of these medicines for me not to have a child, because i been married for 11 months now and my wife is very much possible to conceived a baby but when i had 1st semenalysis last june it was only 24M and my doctor it is still possible but during the 2nd semenalysis it was 0M after a month and taking VIGOR ACE as a supplement, i am surprised as well as my doctor, he said to have another examination but opted not to untill i finish my medication last dec 08, what i want to know if these that i have taken has any side effect for having a low sperm count. is it also bad to be expose to radio active instruments such as ct scan and x-ray, will it damaged the cells and make it difficult to conceive a child? thanks for your immediate reply.

I’m 31 years old, is it OK if i take 1 caps of enduranz in the morning and 1 caps of Rogin-E in the evening? Is it effective to resolve my problem? 4-7 times lang kami ngsisiping ng asawa ko in one month. My problem is after 1 round, lumalambot na agad. Minsan naman pag nagigising ako, sobrang tigas, pero ayaw naman ng isip ko at katawan ko na makipag talik. Minsan naman nasa foreplay plang kami lumalambot na sya.

I’m 27 years old and married for 1 year, and still trying to have a baby. It was found out that my Sperm Motility is less than 50 %, based on the sperm analysis.. Here is the summary of the results of my tests for sperm analysis.
Nov. 8 2008 – Sperm count 39 million / cumm, Motility = 20 %, WBC = 8-12
The doctor advised me to take antibiotic (ofloxacin) because of the WBC count it might be an infection.
After 1 week my urine was subjected to urinalysis. WBC became 0-1. But i was not able to check my motility rate.
January 17, 2009 – I undergo sperm analysis again and Sperm Count = 38.8 mill / cumm, Motility = 40 %, WBC 3-4. I have not consulted yet for a doctor.
Last night February 7, 2009 in Balitang K. I saw a segment there where the husband of Giselle Sanchez having problems with motility. And that enduranz was able to give them their second baby.
My questions are the following
1.Can enduranz help increase my sperm motility ?
2. Can it minimize the formulation of WBC in my sperm ?

Yes, tongkat ali can help as well as increase your sperm count. Clomid has been tried in males to help increase sperm count but it is still up to your doctor if a prescription can be made. Medical follow-up is very important. If you plan to take tongkat ali (Enduranz) please ask from Mercury drug on the cost. It is taken 2x a day: 1 in the morning and 1 in the evening.

Tongkat ali can help increase sperm motility because of the increased availability of the body’s own testosterone. As far as WBC is concerned, seminal fluid will normally contain WBC, especially lymphocytes but if there is no other signs of infection, it may be considered normal. Tongkat ali in its commercial form such as Enduranz is taken two times a day; 1 before breakfast and 1 before supper or 2 hours after supper or at bedtime regardless of whether you are 27 years old or 60 years old. Its effect is gradual and will build up over time, raising the testosterone level, if taken long enough, say 3 weeks. Improving sperm motility and in your case also the number of sperm will raise the chances of fertilization –your first baby.

Tongkat ali which is marketed as Enduranz will usually does not affect your intake of anti-hypertensive and blood pressure. On the contrary men who had maintained their testosterone at almost normal levels is preventive of most illnesses in men such hypertension, diabetes, cholesterol. Delayed ejaculation is thought to be caused by either a problem that can be corrected by surgery if the condition is due to physical factors or by control of anxiety as most of the cases are. It will be best if you consult a urologist to rule out an organic problem (not psychological).

Your heart enlargement could be due to hypertension but you must be able to see a cardiologist for advice. If any medical condition exist, especially the heart, the condition must be control since taking Tongkat ali may make you feel active and can put a strain in your heart if the heart condition has not been treated. It is possible that you may have sperm motility problems. I am not sure if your thyroid condition is related to this unless evaluated by an endocrinologist. There is no problem with taking tongkat ali with Vit C, E and folic acid. These will help improve sperm health. Try to take multivitamins with minerals such as zinc because zinc is a mineral that supports sperm production and agility.

At the moment, I am having a weight loss program. I am taking the capsule called Zheng De Zhou weight loss pill by Magic Potion and i did lose weight. The problem is my erection is not as it used to be. My girlfriend and I are meeting next week, she will be coming back from the US and Im still having problems with my erection, she might think that Im not as good as before.

My husband & I have been married for 1 year & 4 mos. now, & still trying to make a baby. My husband is 37 y/o. We already underwent a work up relative to fertility. It was found out that my husband has minor varicoele, low FSH & testosterone. His prolactin is normal. He has either difficulty extracting sperm whenever he would go on a sperm analysis or when he is able to extract, the lab doesn’t want to analyze it because they say that the amount extracted is little. when he had post ejaculation urinalysis, it was found out that there is rare presence of sperm. hence, we’re still hopeful that we can have a baby. FROM DR EMIL: Don’t worry about the varicocoele if it just minor and does not cause pain. Enduranz (Tongkat ali) can help, take 1 capsule 2x a day before meals. You can check again the circulating or free testosterone (unbound) after 6 months to see how the levels compare with the previous one. We expect that it should go up. Can you post in this blog the original and repeat testosterone (after taking Enduranz for 6 months)?

Note that you need to have adequate intake of fluids/water in order to produce enough semen; tongkat ali (Enduranz) will improve your testosterone for spermatogenesis.

I have found Enduranz to be effective!
Thank you for your blogs.
After taking 2 capsules per day for one month
and three weeks I have been waking up with an
erection for the past week. The erection lasts
from 15 to 30 minutes. Is not prolonged erection
harmful? Do I have to reduce dosage to one capsule
per day?
The other reason I am taking Enduranz is to
increase semen production. On this matter there
is no noticeable improvement. My ejaculation is
still scanty.

FROM DR EMIL: If you are young, say between 17-25 years old and got the results in 2 weeks, you can reduce but if you got the results after 1 month, then 1 capsule 2x is recommended. Your sperm analysis and not seminal volume will indicate better results. The volume of semen depends a lot on fluid intake. Just like urine, if you don’t take enough water, very little water is expected to come out as urine. Try taking 1.5 liters of water daily. Please don’t binge drink water or else you get water intoxication. Try drinking 1.5 glasses of water every 3-4 hours or after meals and in between.

You can try it with 1 capsule once a day since you are still young. There will be increase in sperm count later on. I advise you to take multivitamins with zinc. Prolonged sexual intercourse is up to you since it is more related to full control of ejaculation, which means that you should not be under stress.

Im 27yrs now, and when i was 22yrs i got an std, at may nag advice sakin na uminom ng bactrim at nag patingnin ako sa hospital at nagamot ako may ininject sakin na gamot na pinabnili skin ng doctor. Hi Dr Emil,
I’ve been married for 7 years now and still trying to have a baby. We’ve been to 2 IVF cycles which has been very stressful and financially drained us and no success. My husband has a low sperm count and very low motility. Do you think Enduranz will be able to help him to have his sperm count higher and what do you think would be best for me to take before the next IVF cycle to have a successful implantation of our embryo.

From Dr. Aligui: Your husband may improve this sperm count with Enduranz, but it may take several months before the sperm count can increase. It is worth the try because you might not need IVF in the long run. However, if the problem is poor implantation, then even if there is a successful IVF or if the sperm count is improved, implantation will still be a problem. Given what I said, it would still recommend Enduranz since you could improve fertilization and who knows, you might implant with artificial means.

can enduranz affect the effectiveness of finasteride (Proscar), for hair loss treatment? does enduranz cause baldness or accelerate male pattern baldness?
FROM DR. ALIGUI: Your question is hard to answer since the experience will vary by individual. Baldness can be caused by stress, nutritional disorder, genetics and virility. Virility is a function of testosterone which is a sexual characteristic of males. Dehydrotestosterone (DHT) which comes from testosterone accumulated in the hair follicle, among those who are sensitive to baldness will cause baldness. So theoretically, high testosterone will cause baldness if you have a genetic predisposition to baldness. Since you are already having problems with baldness, Enduranz may contribute to your baldness. However, since Finasteride inhibits DHT activity, even if you increase your testosterone availability by taking Enduranz, Finasteride will still inhibit DHT and theoretically arrest baldness. So my advice is that you must be guided by your doctor who prescribed Finasteride, because if your alopecia is severe, I don’t think you should use Enduranz.

Also avoid using tight underwears, kung di maiwasan dapat mag boxer shorts sa bahay. Avoid sitting on hot areas or seats. Take plenty of nutritious foods –soy, bean sprouts, mushrooms, nuts, multivitamins na may zinc and selenium; avoid stress and have a good rest more often. Avoid too much alcohol and avoid smoking.

Not all males with varicocoele will have fertility problems and even in the presence of varicocoele, some men are able to fertilize an egg. So that’s ok with your bf.

Hi doc, I was quite using enduranz capsule this week ahead for a reason to gain a higher testosterone level, will it help me achieve the muscle mass and increase in size i’ve wanted in a matter of time? please advise. Thank you so much.

From Doc Emil: Hard to say that you can achieve the desired muscle mass in a week. Usually, it takes longer than 6 months and will also depend on your physical activity and training. The report on sports medicine has no specific duration.

doc emil…8 yrs n po kaming kasal ng asawa ko and still wala p po kaming anak…noon nagpacheck-up n po kami and n found out namin n azoospermia ang husband ko…may gamot po b para dito? at kung sakali po n magpaartificial insemination kami…magkano po ang magagastos po namin?
